How we put care into practice

We do not use a one-size-fits-all model. Every person we support starts with a proper assessment. We take the time to understand their history, their preferences, their strengths and their goals before we put any support in place.

1

Referral and assessment

We receive your referral, speak with you and the person needing support, review any existing documentation and carry out a thorough needs assessment.

2

Care plan development

Together with the individual and their network, we build a care plan that reflects their goals and preferences, not just their medical needs.

3

Placement and ongoing support

We match the right staff, start the support, and keep reviewing. As needs change, the plan changes too.
